Requests for Disclosure of Personal Information
In accordance with the Act on the Protection of Personal Information, we will respond to requests from the person concerned or his/her representative for notification of the purpose of use, disclosure, correction, addition or deletion of content, or suspension of provision to third parties (hereinafter collectively referred to as “disclosure, etc.”) of personal information in our possession.
Please note that the following personal information is not subject to disclosure, etc.
- In case there is a risk of harm to the life, body, property, or other rights or interests of the person concerned or a third party
- In case there is a risk of encouraging or inducing illegal or unjust acts.
- In case there is a risk that the prevention, suppression, or investigation of crimes or the maintenance of other public safety and order may be hindered by the disclosure of the existence or nonexistence of the relevant personal information.
- In case we are entrusted with a business and we do not have the authority to disclose the personal information under the contract.
- In case the address on the application form or on the documents for identification does not match the address registered with Sinanen Zeomic, or if the identity of the applicant cannot be confirmed by any reasonable method.
- In case the personal information has already been disposed of or erased
Purpose of using personal information
For the purpose of using personal information, please refer to “2. Acquisition and Purpose of Use” in our approach to personal information protection.
In case the purpose of use of personal information cannot be confirmed in “2. Acquisition and Purpose of Use” above, we will respond to requests for disclosure of the purpose of use.

To request disclosure, etc., please send the prescribed application form with the necessary documents attached to the following address by certified or simplified registered mail or any other method that leaves a record of delivery. Please note that we will not return any documents sent to us.

2. Documents for identification
Please send us a copy of any one of the following. In each case, the copy must confirm the same information as the name and address of the person who entered the information as the subject of disclosure in the Request for Disclosure of Personal Information.
Please submit the following documents with the registered domicile, medical record (insured person's symbol, number, and insurer's number), and basic pension number, if any, masked in the places where they are written.

In case the request is made by the person himself/herself
- 1. Driver's license (if address, etc. has been changed, also the reverse side)
- 2. Passport issued before February 3, 2020 (photo and address page)
- 3. Passport issued on or after February 4, 2020 (photo page) + Certificate of residence (without personal number. The same below) or utility bill receipt
- 4. My number card
- 5. National health insurance card + certificate of residence or utility bill receipt
- 6. National Pension Handbook
- 7. Residence card or special permanent resident certificate

In case the request is made by a proxy
If the person making the Request for Disclosure, etc. is a legal representative of a minor or an adult ward, or an agent entrusted with the request for disclosure, etc., please enclose the following documents (A. or B.) in addition to the documents in (1) above.
A. In case of legal representative
- Documents confirming the right of legal representation
- official copy of the family register
- Copy of adult guardianship registration certificate
- Documents to verify the identity of the legal representative (any of the documents listed in 2.(1) above)
B. In case of a voluntary representative
- power of attorney
- One copy of the person's seal registration certificate (issued within the last three months, with the seal stamped on the power of attorney)
- Documents to verify the identity of the voluntary representative (any of the documents listed in 2.(1) above)
- Documents confirming the right of legal representation
Fees, etc. for Requests for Disclosure, etc.
Please enclose a postal money order for 1,000 yen with the submitted documents.
- No fee is required for requests for correction, addition, deletion, suspension of use, deletion, or suspension of provision to third parties of personal information.
- You are responsible for the cost of purchasing a postal money order and postage to us.
- If the fee is insufficient or not enclosed, we will notify you to that effect. However, if payment is not made within the prescribed period, we will assume that there has been no request for disclosure or notification of purpose of use.
Method of Response to Requests for Disclosure, etc.
Unless otherwise specified in the Request for Disclosure of Personal Information form, we will respond to you in writing or by e-mail to the address or e-mail address provided on the form. Please allow at least two weeks from the time all documents, etc. submitted in response to a request for disclosure, etc. arrive at our company to the time of our response. Please note that there may be a longer delay in responding to your request if there is congestion at the Company's reception desk or other business reasons.
If the set of documents you submitted is incomplete or insufficient, we will return the submitted documents to you by mail with a statement to that effect. Please note that if you do not send the documents by mail again, we will assume that you have not made a request for disclosure.
Purpose of use of personal information obtained in connection with requests for disclosure, etc.
Personal information obtained in connection with a request for disclosure, etc. will be handled only to the extent necessary for the request for disclosure, etc. Please note that the submitted documents will not be returned. The submitted documents will be properly stored and disposed of after the response to the request for disclosure is completed.
